(This form must be completed by state and local governments and presented with agency purchase order to avoid paying 11% federal excise tax).

INDIVIDUAL OFFICER PURCHASE EXEMPTION
(This form must be presented on agency letterhead and bear the signature of an agency supervisor to verify use for law enforcement purposes only).

DESTRUCTIVE DEVICE PURCHASE LETTER
(This form must be presented on agency letterhead and bear the signature of an agency supervisor tin order to purchase WASP-HG™ 20 gauge handgun).

EXPORT DECLARATION
(This form must be presented on company letterhead and bear the signature of an Officer of the Company).
